The album was initially packaged and released as a double vinyl album with seven songs on Side A and seven songs on Side B. The vinyl LP had a cover art designed by the band and contained an inner sleeve with artwork by Anton Corbijn. The cassette tape version had a cover art and contained the same songs as the vinyl release, but without the inner sleeve. Each version of the album was also released as a CD, but the CD versions varied in track listing from the vinyl release. The CD had five songs on Side A and five songs on Side B. The CD also had a miniature booklet with lyrics and photos of the band and artwork by Corbijn.
In Utero was written and recorded as a representation of Cobain's then status. Upon its release, the music industry was caught off-guard by the album's raw sound, and it was a commercial failure upon release. The album gained a favorable critical reception when it was released in 1993, and was eventually certified five times platinum by the RIAA, the highest certified album in the United States, and Cobain was named Artist of the Year by Spin magazine in 1994. Robert Christgau named In Utero the best album of 1993 in his weekly column for The Village Voice, and named it the second best album in the history of the music press in 2011. In Utero has sold 15 million copies worldwide, and many of the album's songs were remixed for inclusion on MTV Unplugged in New York (1993), and the live album Live! Tonight! (1994). In Utero was the band's first release on Sub Pop Records. At the time of its release, the label was not deemed commercially viable, and Cobain left the label to join the band Nirvana for their next album, 1993's Sub Pop 200. Cobain had signed with DGC Records after the dissolution of Nirvana's previous label, Geffen Records, which had signed the band in 1989.
Nirvana's music and press dominance defined the beginning of the alternative rock genre and the subsequent grunge movement. In Utero is considered by many to be a seminal record of the 1990s. In December 2011, Rolling Stone ranked the album at number 1 on its list of the greatest albums of the past 35 years. In 2012, In Utero was named the greatest album of the past 35 years in a Q magazine poll. In 2013, NME ranked the album at number 2 in its list of the 500 Greatest Albums of All Time. In 2016, the magazine listed the album as the 4th best album of the 1990s. In 2019, Time magazine listed the album as the 21st best album of the 1990s. 827ec27edc
